In addition to a photocopy in Alfred R. Young's papers, photocopies were provided by Anton F. Bilek and Raymond J. Patenaude. Tony's copy was obtained in 2006. Ray's 1991 photocopy accompanies a letter and other material sent to Alfred R. Young on December 5, 1991. Ray sent the 1996 photocopy along with a letter and other items in September 1996. Each of these photocopies is housed in The BenHaven Archives along with associated correspondence.

Patenaude appears on three 8.5 in. x 11 in. sheets of paper. The 1996 photocopy is also on letter-sized paper and consists of only two sheets (two pages of the original document are missing). The photocopy found in Alfred R. Young's files appears on 8.5 in. x 11 in. sheets of paper.
